The fastest and most convenient option to request a ballot is to apply on line from the Secretary of State’s Office:

  • Save the .pdf file to your computer.

  • Complete the application.

  • Save and print the application.

  • Sign the application (We cannot accept applications without a signature.)

  • You may U.S. Mail your signed application back to us, or scan your signed application and electronically submit it to us.
